General Service List

The following information is effective as of December 15, 2000. And are subject to change without notice.

The Federal Trade Commission (FTC) and the New York State Department of Health require funeral providers to make certain disclosures on the General Services List. These disclosures are similar, but are not exactly the same.

This General Services List contains the disclosures required by the New York Stated Department of Health and the New York City Department of Consumer Affairs.

  • The goods and services shown below are those we can provide our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our services. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral service and merchandise you selected.
  • This list may not include prices for certain items select, such as cemetery charges or crematory services, flowers and newspaper notices. The estimated or final prices for those items will be shown on the itemized statement describing the funeral services and merchandise selected. We charge you for our services in buying certain items.
  • Your authorization is required for a funeral home to obtain custody of a body, and a body must be released promptly upon request.
  • The provider of funeral services is required to give price information and over the telephone.
  • A provider of funeral services is prohibited from representing that the deterioration of human remains can be prevented for more than a limited time by embalming, caskets, vaults or outer interment receptacles.

ALTERNATIVE SERVICES:

Direct Cremation
Direct cremation includes the local transfer of the remains to the funeral firm, the services of the funeral director and staff, securing of necessary authorizations, basic local transportation to the crematory, an alternative container for cremation and the return of the cremated remains to the funeral firm.

We offer a selection of 6 alternative containers including containers made of wood, containers made of fiberboard and containers made of other composite materials.

In all, we offer 9 basic package services for direct cremations:

  • Direct cremation with a cremation container supplied by the purchaser
  • Direct cremation with a chipboard alternative container(earth brown exterior finish)
  • Direct cremation with a fiberboard alternative container (mustard/brown exterior finish)
  • Direct cremation with a medium density fiberboard alternative container (frost oak exterior finish)
  • Direct cremation with a pine veneer alternative container (clear gloss exterior finish)
  • Direct cremation with a poplar wood alternative container (polished walnut exterior finish)
  • Direct cremation with a cherry wood alternative container (polished cherry exterior finish)

If you want to arrange a direct cremation, you may use an unfinished wooden box or an alternative container. Alternative containers can be made of heavy cardboard, pressed-wood, or composite materials (with or without an outside covering) or may be pouches of canvas or other materials.

Direct Burials
We offer two basic package services for direct burials:

  • The first package includes transfer of the remains to the funeral firm, services of the funeral director and staff, securing the necessary authorizations, basic local transportation to the cemetery and the Alternative Container.
  • The second package includes the services as described above but with container supplied by the customer.
Transfer of Remains to the Funeral Firm including Personnel, Equipment, and Vehicles
  • Local transfer within 10 mile radius
  • Charge per mile beyond local range
  • Additional assistance (for home or certain hospital/nursing home transfers)-per man
  • Medical School Transfer (each transfer)
Preparation of Remains
  • Embalming
  • Embalming of autopsied remains
  • Embalming of remains at private residence
In New York State, embalming is not required by law. If you do not want embalming, you have the right to choose an arrangement that does not require you to pay for embalming such as direct cremation or direct burial. If you select certain funeral arrangements, such as viewing or an open casket funeral, embalming may be required by the funeral firm.

Other Prepartion
Preparation of remains, unless selected, is not required for direct cremation or direct burial.

  • Topical Disinfection
  • Custodial care (charge per day after 6 hours) Custodial care will not be charged on the days when embalming or other preparation of remains occurs, visitation is held or a funeral service is conducted.
  • Dressing / casketing of remains
  • Cosmetology
  • Restoration (per hour)
  • Hairdressing
  • Other: Refrigeration (required by the funeral firm after 6 hours)

Arrangements
The charge for our services will be added to the cost of the funeral arrangements you select. However, this charge will not be added to the cost of direct cremation, direct burial, forwarding of remains or receiving of remains because the prices for those include this charge.

BASIC ARRANGEMENTS: Include funeral director, other staff, equipment and facilities to respond to initial request for service, the arrangement conference, securing of necessary authorizations and coordination of service plans with parties involved in the final disposition of the deceased.

The Federal Trade Commission prohibits the provider of funeral services from imposing any additional fee or surcharge to consumers who obtain a casket elsewhere.

LIMITED SERVICES

Forwarding Remains to Another Funeral Home:
This service includes the transfer of the remains to the funeral home, staff services, securing necessary authorizations and local transportation to the point of shipment. The service does not include shipping or additional items of service, merchandise, facility use and livery that the consumer may wish to select.

Transportation beyond the local point of shipment will be billed as either livery or common carrier charges depending upon the forwarding location. Local point of shipment is up to or within 10 miles.

Receiving of Remains from Another Funeral Home:
This service includes the basic transportation of the remains to the funeral home and or place of local disposition, staff services and securing necessary authorizations. This service does not include additional items of services, merchandise, facility and use of livery that the customer may wish to select.

Transportation beyond the local point of disposition will be billed as livery charges. Local point of disposition is up to or within 75 miles.

Services and merchandise provided as cash advance items will be billed at the same amount that is paid by the funeral home. These are:
